Tax Obligations for Clinics in the UAE: Finance, Tax and Operations Guide
Short answer: The main tax obligations for clinics in the UAE are corporate tax, VAT and paying staff on time through the Wages Protection System. Corporate tax is 0% on taxable income up to AED 375,000 and 9% above it. Most patient care is zero rated for VAT, but a clinic still registers once its taxable supplies pass AED 375,000. Salaries are due through WPS by the first day of the following month. This page explains how the three fit together and links every finance guide in our library.
Last verified: October 2026 against the FTA pages on VAT registration and Small Business Relief, the u.ae corporate tax page, Ministerial Decision No. 131 of 2026 and MOHRE Ministerial Resolution No. 340 of 2026.
Key Takeaways
- Healthcare has no special corporate tax rate: 0% on the first AED 375,000 of taxable income and 9% on the rest, with a return due within nine months of the year end.
- Small Business Relief is open to businesses with revenue of AED 3 million or less, and the Ministry of Finance has extended it to tax periods ending on or before 31 December 2029.
- Zero rated patient care still counts as a taxable supply, so it counts toward the AED 375,000 VAT registration threshold and keeps your right to recover VAT on costs.
- Since 1 June 2026, salaries paid after the first day of the month count as late under WPS, and MOHRE’s measures escalate within days.
- Most clinic tax problems start with set up decisions: who holds the licence, how doctors are paid and whether cosmetic work is billed correctly.

The Three Tax Obligations for Clinics at a Glance
Clinic owners often treat tax as a year end job for the accountant. In practice the tax obligations for clinics run all year: VAT every return period, payroll every month and corporate tax shaped by the accounts you keep from day one.
| Obligation | The Core Rule | Where To Read More |
|---|---|---|
| Corporate Tax | 0% up to AED 375,000 of taxable income, 9% above it; return and payment within nine months of the year end | Corporate Tax for Clinics |
| VAT | Mandatory registration above AED 375,000 of taxable supplies; most healthcare by a licensed provider is zero rated, cosmetic work without a prescription is 5% | VAT on Healthcare Services |
| Payroll and WPS | Wages through WPS by the first day of the following month; at least 85% paid on time only counts where the gap is a lawful deduction | Payroll and WPS for Clinics |
| End of Service | Gratuity builds up every month of service and should be accrued, not left for the exit | UAE Gratuity Calculator |
Corporate Tax: Who Pays and When
A clinic run through a company, mainland or free zone, is a taxable person from its first tax period. A doctor who holds the clinic licence in their own name is taxed as a natural person once business turnover passes AED 1 million in a calendar year, and a clinician’s salary from employment is outside corporate tax altogether. The FTA confirms that the return and any tax due are filed within nine months of the end of the tax period.
Small Business Relief helps smaller practices. It needs revenue of AED 3,000,000 or less in the current and every earlier tax period, and the FTA says a business claiming it must still file a return. In August 2026 the Ministry of Finance issued Ministerial Decision No. 131, which extends the relief to tax periods ending on or before 31 December 2029. Our corporate tax guide covers the registration deadlines, the late registration penalty and the costs that move a clinic’s tax bill most.
VAT: Zero Rated Does Not Mean Exempt
The FTA’s registration page sets the mandatory threshold at AED 375,000 of taxable supplies and imports, and the voluntary threshold at AED 187,500. Because zero rated healthcare is still a taxable supply, a busy clinic usually crosses the line even if almost nothing it bills carries 5%.
Three areas cause most VAT errors in clinics. Cosmetic treatment that no doctor prescribed is standard rated. Services one provider sells to another, such as lab work or a visiting specialist billed to a hospital, are standard rated because the patient is not the customer. And retail products sold over the counter are only zero rated if they appear on the Cabinet list. The VAT on healthcare services guide has a line by line table of what a typical clinic sells.
Payroll: The Monthly Deadline That Bites First
Of all the tax obligations for clinics, payroll is the one with the shortest fuse. MOHRE Ministerial Resolution No. 340 of 2026 names the first day of each Gregorian month as the single due date for last month’s wages, and says any payment after that date is a delay. Alerts start from the second day, and from day five MOHRE can stop new work permits, which can freeze hiring just when a clinic is trying to fill a roster.
For clinics the hard part is the mix of full time, part time and locum clinicians, each needing the right permit, contract and professional licence before they reach payroll. Our payroll and WPS guide walks through the escalation table and the paperwork for staff who are excluded from WPS.
Running the Numbers Between Tax Deadlines
Tax filings only show what already happened. The clinics we see in trouble usually had the warning in their cash flow months earlier: insurers paying weeks or months after the visit while salaries are due on the first. A rolling cash forecast, monthly management accounts and a clear view of insurance receivables do more for a clinic’s tax position than any year end planning.

Frequently Asked Questions.
What taxes do clinics pay in the UAE?
A clinic company pays corporate tax at 0% on taxable income up to AED 375,000 and 9% above it. It registers for VAT once taxable supplies pass AED 375,000, even though most patient care is zero rated. Salaries go through WPS by the first day of the following month.
Do clinics need to register for VAT if healthcare is zero rated?
Usually yes. Zero rated supplies are taxable supplies, so they count toward the AED 375,000 mandatory threshold. Registering also lets the clinic recover VAT paid on rent, equipment and other costs.
Can a small clinic still use Small Business Relief?
Yes, if its revenue is AED 3 million or less in the current and every earlier tax period. Ministerial Decision No. 131 of 2026 extends the relief to tax periods ending on or before 31 December 2029, and a return must still be filed to claim it.
When must a clinic pay salaries under WPS?
By the first day of the following Gregorian month. Since 1 June 2026, Ministerial Resolution No. 340 of 2026 treats any payment after that date as late, and MOHRE can suspend new work permits from day five of a delay.
Is a doctor’s salary subject to corporate tax?
No. Wages from employment are outside corporate tax. A doctor who runs a clinic in their own name is taxed as a natural person once business turnover passes AED 1 million in a calendar year.
